Fees & Superbills

This practice accepts self-pay clients, provides superbills for out of network benefits, and is an in-network provider with most commercial Aetna plans. 

 Please note that Medi-cal & Medi-care plans may have additional restrictions that limit reimbursement.

We accept major credit cards and Zelle.​

Checking Out of Network Coverage

Clients are responsible for checking their individual benefits. Before scheduling, please follow these steps.

1.) Please call the member services number on the back of your card.  Remember to get the name of the representative you spoke with and ask for a call reference number. Ask these questions:

  • Do I have out-of-network benefits? Many HMO plans do not cover out-of-network care at all, while PPO plans typically do. 

  • What is my out-of-network deductible? This is the separate amount you must pay out-of-pocket for non-network care before your insurance starts reimbursing you. 

  • What is my coinsurance or reimbursement rate? For example, the plan might pay 60% of their "allowable amount" for out-of-network services, leaving you to pay the remaining 40%.

  • What is the "Allowed Amount" (or UCR)? Insurers base reimbursements on what they consider a "reasonable" fee. If your provider charges $200 but the allowed amount is $150, you will be responsible for the $50 difference.

   

2.) Once you've verified out-of-network benefits, follow this process:

  • Pay upfront: Out-of-network providers require you to pay the full cost of the visit at the time of service. 

  • Request a Superbill: Ask your provider for an itemized receipt, called a superbill, which includes codes for your diagnosis and the exact services rendered. 

  • Submit the Claim: Submit the superbill along with a claim form via your insurer's online portal, app, or by mail to get reimbursed for the covered portion.

Workshop costs will be calculated based on time, cost of materials, preparation, and mileage. 

Please contact me with your desired workshop including times and dates, topic or theme, estimated number of participants, desired activities and materials, and information about the venue and space with photos.

No Surprises Act

You have the right to receive a “Good Faith Estimate” explaining how much your medical and mental health care will cost.

Under the law, health care providers need to give patients who don’t have insurance or who are not using insurance an estimate of the expected charges for medical services, including psychotherapy services. 

You have the right to receive a Good Faith Estimate for the total expected cost of any non-emergency healthcare services, including psychotherapy services.

You can ask your health care provider, and any other provider you choose, for a Good Faith Estimate before you schedule a service.

If you receive a bill that is at least $400 more than your Good Faith Estimate, you can dispute the bill. Make sure to save a copy or picture of your Good Faith Estimate.

Out of Network Benefits: Mentaya
